NNU Membership Application - Payroll Deduction Authorization Form

This year's 2011 NNU membership drive has been spectacular. We have gathered well over 1,400 Dues Forms from Registered Nurses at WHC. That makes our Union 1,400+ nurses stronger! We still need to gather a few more Forms from you to get all of us on board with membership. Please take a moment to download the NNU Membership Application - Payroll Deduction Authorization Form , print it out, and complete it. Make sure to sign BOTH signature lines on the Form, then mail it back to us or fax it to us. Dues remain the same - unchanged since 2003.
